Fast Track Medical Care is required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our legal duties and privacy practices, and notify you in the event of a breach of your unsecured PHI.
We are required to follow the terms of this notice. We reserve the right to change our privacy practices and make the new practices effective for all PHI we maintain. If we make a material change, we will post the updated notice on our website and make it available at our clinic.
What Is Protected Health Information (PHI)?
PHI includes any information we create or receive about your past, present, or future health condition; the provision of healthcare to you; or payment for healthcare services — when that information can be used to identify you.
How We May Use and Disclose Your PHI
Without your authorization, we may use and disclose your PHI for the following purposes:
- Treatment — sharing information with physicians, nurses, or other providers involved in your care
- Payment — billing your insurance company or processing payment for services
- Healthcare operations — quality improvement, staff training, audits, and administrative functions
- Public health activities — reporting communicable diseases, vital statistics, or adverse events as required by law
- Law enforcement — responding to court orders, warrants, or lawful requests from law enforcement agencies
- Emergency situations — disclosing information to prevent serious threats to health or safety
- Workers' compensation — as authorized by and to the extent necessary to comply with workers' compensation laws
Any use or disclosure of your PHI beyond those listed above requires your written authorization. You may revoke that authorization at any time in writing, except where we have already taken action based on it.
